How to Find the Best Doctor for Liver Transplant in India for Complex Cases

If you’re trying to find the best doctor for liver transplant in India, most of what turns up is the same handful of large hospital names, with little to actually separate one surgeon from another. This information doesn’t tell you much about who’s actually good with a difficult case. And that distinction is important far more once you’re dealing with a complex liver transplant. A re-do transplant, a transplant with portal vein thrombosis, or a paediatric case doesn’t get the same margin for error as a standard living donor transplant with a low MELD score. Here’s what actually distinguishes a surgeon suited to a difficult case from one suited to a routine one. Difficult liver transplant cases like these need a surgeon evaluated on different criteria altogether, and that’s what this piece is really about.

What actually makes a case “complex”

A few things push a transplant out of the routine category:

    • A high MELD or PELD score, where the patient is already quite sick going into a long surgery
    • Portal vein thrombosis, where the main vessel feeding the liver is blocked, and the vascular reconstruction has to be planned differently
    • Re-transplantation, where scar tissue from a previous surgery makes the anatomy harder to work with
    • ABO-incompatible donors, which need a desensitisation protocol before transplant
    • Small-for-size graft risk in living donor cases, where the donated portion isn’t quite large enough for the recipient
    • Paediatric liver transplant, particularly in infants and low-weight children, where vessel reconstruction and post-operative care look quite different from an adult case
    • Combined organ involvement, such as a liver-kidney transplant, or significant cardiac or lung disease alongside liver failure

None of this means transplant isn’t possible. It means the team taking on the case needs to have actually handled that specific complexity before, not just a high transplant count in general.

Why the surgeon matters more once a case gets complicated

For a routine transplant, most experienced surgeons in India will get a good outcome, and the differences between centres are smaller than people assume. Complex liver transplant surgery is where that changes. It shows up in how blood loss is controlled, how the vascular anatomy is reconstructed, and whether complications are anticipated ahead of time or managed after they’ve already happened.

So when looking for the best liver transplant specialist in India for a difficult case, the question isn’t just “how many transplants has this surgeon done?” It’s whether they’ve dealt with cases like this one.

What to actually ask when evaluating a surgeon

Ask about their experience with the specific complexity involved. A surgeon with several hundred transplants behind them may still have handled very few re-transplants or PVT cases. Ask directly how many cases similar to this one they’ve managed, not just their overall number.

Check their HPB training background. Liver transplant sits within hepatobiliary and pancreatic surgery, and a surgeon with dedicated HPB fellowship training tends to have a firmer grasp of the vascular and biliary anatomy that complex cases depend on.

Look past the surgeon to the team. A hepatologist managing pre-transplant optimisation, an anaesthesiologist used to long liver transplant cases, and an ICU set up for post-transplant recovery matter almost as much as the surgeon’s own hands. This isn’t a one-person job.

If the patient is a child, ask specifically about paediatric experience. Paediatric liver transplant needs a team comfortable with small-vessel reconstruction and weight-adjusted immunosuppression: it’s a different skill set from adult transplant work, not a scaled-down version of it.

Understand how living donors are evaluated. Deceased donor organs are still limited in India relative to demand, so most transplants here go the living donor route. In complex cases, donor-recipient size matching and graft volume calculation matter even more, so it’s worth asking how the team works through donor selection.

Don’t treat “inoperable” as final without a second opinion. Cases turned down at one centre are sometimes taken on elsewhere with a clear surgical plan. A second consultation, ideally with a surgeon who regularly handles that type of complexity, is worth the time before ruling transplant out.

    About Dr. Prashant Kadam

Dr. Prashant Kadam is a hepatobiliary and liver transplant surgeon based in Mumbai, with over 15 years of experience. He completed his surgical training at Sion Hospital, Mumbai, before spending five years at Queen Elizabeth Hospital, Birmingham, in the UK. He handles some of the harder liver transplant cases, such as repeat transplants, cases where a major blood vessel near the liver is blocked, and transplants where the donor and patient’s blood types don’t fully match. He’s also trained in paediatric and robotic liver transplant surgery, along with newer donor liver preservation methods such as machine perfusion. Each case is planned around its own vascular picture rather than a fixed protocol, with a team of hepatologists, anaesthesiologists, and intensive care specialists involved from donor workup through recovery.

    Frequently Asked Questions

Q1. What is considered a complex liver transplant case?

Cases involving portal vein thrombosis, re-transplantation, ABO-incompatible donors, very high MELD/PELD scores, or paediatric patients are generally considered complex, since they need additional surgical planning beyond a standard transplant.

Q2. Is a living donor transplant possible in complex cases?

In many cases, yes. Feasibility depends on the specific complexity involved and the donor’s suitability, which is assessed through detailed imaging and clinical evaluation before donor approval.

Q3. How is a paediatric liver transplant different from an adult transplant?

An adult transplant deals with larger blood vessels and bile ducts, and immunosuppression doses are set for a fully grown body. A paediatric transplant is different on all three counts: the vessels and bile ducts being reconstructed are much smaller, medication doses are calculated by the child’s weight and adjusted as they grow, and post-operative monitoring has to account for a child’s physiology, which responds differently to surgery and medication than an adult’s does.

Q4. Can a case that was declared “inoperable” at one hospital be reconsidered elsewhere?

Yes, this happens often enough that a second opinion is worth pursuing. Different centres may have different experience levels with the specific complexity involved in a case.

Q5. What questions should be asked before choosing a surgeon for a complex transplant?

Ask about their direct experience with the specific complexity involved (not just overall transplant numbers), their HPB training background, and the multidisciplinary team supporting the transplant and post-operative care.
